Job Details

• Evaluate, plan, and implement evidence-based treatment programs
• Collaborate with interdisciplinary teams to ensure holistic care
• Track patient progress, adjust care plans, and deliver outcomes-focused therapy
• Educate patients and families on exercises, prevention, and long-term recovery
• Maintain accurate and timely clinical documentation
