About The Position
Job Title: Home Health Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 2 years of experience required License: State license required Certifications: BLS, OASIS, MI state license required Must-Have: - Required experience in home health and hospice - Experience with adults and geriatrics - Familiarity with Kinnser software Description: Bed Side Shift Reporting (BSSR), utilization of AIDET, and hourly rounding are required for this position. An ASAP start is required. This role involves working with home health and hospice patients. You will be required to work every 4th or 5th weekend. On-call evening rotation is expected a few times per month. There is no holiday time off allowed; holidays will be scheduled as needed. Candidates will float within the facility as needed. Interdisciplinary support includes physical therapy, interpretation services, and social services. You will manage patients with various diagnoses related to home health and hospice. Experience in admission assessments, medication administration, pain management, and wound care is essential. Must be skilled in conducting clinical assessments and documentation. A thorough understanding of infection control precautions is necessary. You will coordinate services and manage discharge planning. The ability to obtain vital signs and perform basic nursing procedures is required. Management of feeding tubes and urinary catheters is expected. Familiarity with pain scales, medication assessment and management, and G-tube care is required. You will be responsible for ensuring the safety and compliance of patients. Comfort in managing various aspects of patient care, including equipment such as peripheral IVs and central lines, is critical. The role demands high levels of communication skills and compassion while interacting with patients and families.
