*Work Requirements:*
Private Duty Home Health Nurse must hold a current NYS Registered Nurse license. Possess organizational skill, attention to detail, ability to act autonomously and problem solve. Must have a valid drivers’ license, pass a background check and have required immunizations. Must have reliable transportation to and from patients’ residences for work assignments. High degree of confidentiality required.
*Position Summary:*
Manages the nursing needs of assigned patients from admission to discharge ensuring patients receive quality care. The Private Duty Home Health Nurse practices his/her duties in a manner that is compliant with federal, state, and local codes, rules, regulations, and law. Private Duty Home Health Nurse will practice competent clinical skills and perform timely accurate documentation as evidenced by clinical records audits and supervisory visits and chart reviews. The Private Duty Home Health Nurse will communicate to patients and families information on the disease process and treatments.
*Essential Job Duties:*
1. Performs nursing assessments of patients per the plan of care and notifies the care managing nurse of any problems or changes in patient status.
2. Prepares and applies dressings, prepares and administers medication, and performs nursing treatment as prescribed by an authorized practitioner.
3. Cares for patients in their living quarters, including assuring personal hygiene, preparing and serving meals, assisting with feedings, transferring and positioning, bathing and skin care, assessing patient's health status, and performing other patient care as ordered.
4. Assists with the rehabilitation of patients in accordance with the nursing care plan, such as assisting with ambulation and joint range of motion, applying the principles of good body mechanics and alignment.
5. Instructs the patient or patient’s caregiver regarding patient’s diet, health habits, treatments, and rehabilitative measures as ordered.
6. Obtains and documents interim orders as appropriate and notifies the Director of Patient Services in a timely manner of such orders.
7. Communicates with the physician as directed by the Director of Patient Services.
8. Participates in the formulation of the patient’s total plan of care, and confers with other team members as needed.
9. Supervises aides as needed to assure quality care.
10. Records on patient charts all evaluations and progress notes pertinent to each nursing visit/shift, by the end of the day in which the visit was made. Submits documentation on a weekly basis.
11. Participates in patient care conferences and agency meetings.
12. Adheres to HIPAA regulations governing confidentiality and security of protected health information for both manual and electronic patient records, charts, and other confidential information such as employee records, computer access codes and passwords.
13. Performs other appropriate duties, as needed.
*Physical/Mental Demands:*
* Physical demands may include but are not limited to lifting, bending, standing and transferring patients (with equipment when necessary).
* Mental demands may include but are not limited to emotions of grief upon learning of a decline in the medical condition or the passing of a patient.
*Working Conditions:*
* Home Care field work involves increased risk while driving during inclement weather to and from patients home and other work assignments.
* May work in non-sterile, unpredictable environment (patients household items, icy walkways) including exposure to animals. Increased exposure to infectious diseases; must follow established protocols.
* Some irregular hours may occur and adjustments made to work assignment for the welfare of the patient.

Pay: From $34.50 per hour
