*Responsibilities:*

· Provide comprehensive rehabilitation services to members of the community.

· Evaluates patients by conducting a thorough assessment and by applying diagnostic and prognostic functional tests.

· Develops reasonable, specific and measurable goals that support the plan of care, are agreed upon by the patient/caregiver.

· Communicates with MD, family and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.

· Provide patient and family with appropriate information and materials necessary to support the learning process, i.e., home exercise program sheets and instruction sheets. This position will allow you to utilize and develop your skills in case management.

Job Types: Full-time, Part-time

Pay: $90,000.00 - $103,000.00 per year
