The purpose of positions in this class is to provide outreach and care management services to individuals in need and their families by assessing participant needs, providing information and referrals on resources, determining participants' qualification for various available programs, and developing, coordinating and monitoring care plans.
The work typically is performed under the supervision of clinically licensed staff in higher classifications.
This position is Extended Part-time with a schedule requirement of up to 32 hours per week.
The ideal candidates will possess equivalent to two years of college or university level work in social work, gerontology, human services, social ecology or closely related field, and two years of social work, outreach or counseling, or any combination of education and experience that provides equivalent knowledge, skills and abilities. The Process Interview: After a thorough review of applications, those candidates deemed most qualified as reflected in their application will be invited to an interview.
Background Check: A thorough background investigation will be conducted prior to appointment.
Medical Examination: A medical examination will be conducted, including a drug/alcohol screening and proof of a negative TB test to ensure the ability to perform assigned duties. Any applicant who tests positive or tampers with evidence shall be disqualified from consideration.
