Job Description

The Clinical Documentation Specialist is responsible for facilitating improvement in the overall quality and completeness of medical record documentation.The Clinical Documentation Specialist obtains appropriate clinical documentation through auditing processes and extensive interaction with physicians, nursing staff, other patient caregivers, and coding personnel to ensure that documentation accurately reflects the level of services rendered to patient, and that the clinical information utilized in profiling and reporting outcomes is complete and accurate.The Clinical Documentation Specialist actively provides education to all members of the patient care team on documentation guidelines and follow through based on audit results.

Education Required : Must be a Licensed Registered Nurse or Licensed Practical Nurse in the state of Wisconsin.

Related Years of Experience Required: Three years of recent clinical experience in a hospital or outpatient clinic setting is preferred.Experience within quality improvement, provider quality audits, and provider education is preferred.

Credential/Licensure Required : CCDS through ACDIS, or CDIP through AHIMA, preferred. Additionally, one of the following: LPN or RN with strong coding background (RHIT, RHIA, CCS or CCS-P).

Shift: Working 40 hours a week, Monday - Friday.
