About Americares Free Clinics :
The mission of the Americares Free Clinics is to provide quality health care to low-income, uninsured patients in a setting where all individuals are treated with dignity and respect. Americares opened its first clinic in 1994 to expand access to care for the working poor. Over the years, our program has grown to become the largest free clinic network in the state with four locations serving 2,000 patients annually. Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.

About the position:
The Adult Nurse Practitioner (NP) will work in collaboration with the medical team, Clinic Director, and administrative suppo rt . Our target population is adults, the majority of whom have at least one chronic condition requiring careful follow up. The NP should be skilled in counseling patients on preventive care and be comfortable prescribing medications for chronic and acute conditions. A knowledge of motivational interviewing , resource stewardship and resource utilization are desirable.
Key Outcomes:
In the first 90 day s , the NP will
- Learn and live the Americares values.
- Complete all required new hire onboarding trainings .
- Demonstrate an understanding of the patient population and demographics.
- Complete Athena (EHR) learning modules , incorporate all aspects of medical documentati on and use the software appropriately with little direction.
- Demonstrate respect for patients and staff while delivering quality care
- Demonstrate an understanding of clinic policy and procedures.
- Utilize resources to patients’ best benefit.
- Understands patient population and demographics.
In the first 6 month s, the NP will:
- Demonstrate f ully functional competency in Athena , including fulfilling preventive care guidelines.
- Deliver high quality medical care to patients as demonstrated in peer review.
In the first yea r , the NP will:
- Participate in peer review and quality programs
- Participat e in policy development as requested
- Work collaboratively with team to promote new programming and approaches to efficiency and quality care.
Duties and Responsibilities :
- Provide a thorough and appropriate medical exam and render proper care as required in the diagnosis and treatment of patients within the limits of experience and professional training .
- Provide appropriate patient instruction and/or consultation with family or significant other, as needed.
- Use of Athena Electronic Health Record to document examination, diagnosis, and treatment notes . Provide clear orders, and be available to nursing staff for direction, support, and clarification of the medical plan of care.
- Appropriately utilize diagnostic and laboratory services as required for quality care; identify those patients beyond our scope of care; make patient referrals to appropriate specialty services; determine medication needs utilizing available resources.
- Provide care to patient according to primary medical home model including following preventative care guidelines and participating in quality improvement activities.
- Serve as a consultant with regard to decisions pertaining to medical policy and patient care, and performance standards.
- Model exemplary professional behavior in a team building atmosphere respecting the roles and responsibilities of entire team
- Acknowledge that AFC maintains a policy and procedure manual at all locations and agree to follow the policies and procedures set forth within the manual.
- Actively model and contribute to Americares values, work culture and mission.
- Engage in and contribute to team spaces with openness, global competencies, and a growth mindset
- Other duties as assigned
Degrees and/or experience required for the role:
- Master’s degree and l icense from accredited nurse practitioner program with certification in adult medicine or family practice and able to work independently in the State of Connecticut
- At least two years of experience as NP providing care to adults with chronic disease.
- Experience working in primary care settings with an adult patient population
- Experience in supporting patients with chronic health care conditions
These competencies are required for the role:
- Demonstrated ability to m aintain current knowledge of treatment protocols
- Excellent communication skills, including verbal and written, and ability to give and receive feedback
- E xcellent bedside manner and patient support
- Experience and ability to work with a medical interpreter, as needed
US work authorization is required for this role.

- Fluency in Spanish in medical contexts
- Experience working with a culturally diverse and underrepresented population
